Transportation Vouchers Help Montezuma County Residents Reach Medical Appointments

  • Local Champion: Axis Health System
  • Date Funded: May 24, 2022
  • Grant Amount: $2,250

Many families and individuals in Montezuma County don’t have access to a car or to reliable transportation, which is a major problem when it comes to keeping and attending urgent medical appointments. With this in mind, a community health worker at Axis Health System reached out to LOR with a solution: purchase vouchers for the Montezuma County Public Transportation for individuals in need, helping them reach their appointments at Axis Integrated Health and other providers. To streamline the process, EMTs and clinicians working with Montezuma County’s new community intervention program will pass out the vouchers and help connect residents with additional resources.
